The DHHS Community Care Program, part of the Office of Population Health Equity, is dedicated to improving access to essential health programs and social services for all residents of Fairfield, ME, and beyond. Through our user-friendly online referral system, we guide individuals facing barriers in accessing the support they need. By filling out our simple referral form, participants gain access to a wide range of resources tailored to their needs, including culturally and linguistically appropriate services, maternal and child support, healthcare navigation assistance, health and wellness education, as well as options for food, heating, and transportation. We also provide resources to help manage stress and anxiety and offer COVID-related support. After submitting a referral, you can expect a prompt response from one of our partner organizations within 1-3 days to discuss your specific needs and explore available options.
