At the DHHS Community Care Program in Fairfield, ME, we are dedicated to bridging the gap between Mainers and essential health programs, social services, and resources. Our mission is to empower individuals, especially those facing barriers in accessing services they qualify for, through a user-friendly online referral system. By filling out our referral form, community members gain access to a wide array of resources, including culturally and linguistically appropriate services, maternal and child health support, guidance through the healthcare system, health education, and assistance with food, heating, and transportation needs. We also address mental health concerns with support for stress and anxiety, as well as resources related to COVID-19. Once a referral is submitted, our compassionate staff will connect individuals with the right organizations, typically within 1-3 days, ensuring that they receive the help they need. We believe in fostering a supportive environment where everyone can thrive, and we are committed to serving all of Maine with the utmost care and respect.
