The DHHS Community Care Program, part of the Office of Population Health Equity in Fryeburg, ME, is committed to ensuring that all Mainers can access essential health programs, social services, and resources tailored to their needs. Our inclusive program addresses the barriers many individuals face when seeking support, offering assistance from navigating the healthcare system to access for culturally appropriate services, maternal and child resources, health and wellness education, and emergency aid for food, heating, and transportation. Our dedicated team facilitates connections through an easy-to-use online referral system, pledging to respond within 1-3 days to help individuals explore their options. Each household can submit one referral, and while services vary by region, we strive to provide timely and pertinent support to enhance the well-being of our community members during challenging times.
