The DHHS Community Care Program, part of the Office of Population Health Equity in Gray, Maine, is dedicated to ensuring that all residents can access vital health programs and social services, especially those facing barriers. Our online referral system simplifies the connection to a diverse array of resources tailored to individual needs, including culturally and linguistically appropriate services, maternal and child resources, navigation assistance in the healthcare system, health and wellness education, and options for food, heating, and transportation. We also offer support for managing stress and anxiety and provide COVID-related resources. By submitting a referral form, each household can initiate contact with our team, who will connect them with partner organizations within 1-3 days.
