The DHHS Community Care Program, part of the Office of Population Health Equity in Greene, ME, is dedicated to breaking down barriers so every Mainer can access vital health programs and social services. Through our user-friendly online referral system, we connect individuals to a wide range of essential resources, including culturally and linguistically appropriate services, support for mothers and children, healthcare navigation assistance, health education, and critical resources for food, heating, and transportation. We also offer help for stress, anxiety, and COVID-related concerns. By simply filling out our referral form—one submission per household—community members will receive personalized assistance. Our dedicated staff will reach out within 1-3 days to discuss specific needs and connect individuals with appropriate organizations.
