The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC, is dedicated to improving access to essential health programs and social services for all Mainers, especially those facing barriers. Our online referral system connects individuals and families in Greene, ME, with a wide array of resources tailored to their needs. From culturally and linguistically appropriate services to support for mothers, babies, and children, we offer assistance navigating the healthcare system, health and wellness education, and options for food and transportation. We also provide resources for managing stress and anxiety, as well as COVID-related support.
