The DHHS Community Care Program, part of the Office of Population Health Equity in Holden, ME, is dedicated to connecting Mainers with essential health programs, social services, and various resources tailored to their needs. With a focus on inclusivity, this program helps those who may face barriers to accessing the support they qualify for. By filling out our easy online referral form, individuals can access a wide range of resources, including culturally appropriate services, assistance for mothers and children, healthcare navigation help, health and wellness education, and options for food, heating, and transportation support. Additionally, we address mental health concerns such as stress and anxiety and provide COVID-related resources. Once a referral is submitted, our team will work promptly—usually within 1-3 days—to connect individuals with partnering organizations that can assist with their specific needs.
