Located in Holden, ME, the Living with Type 2 Diabetes Program is dedicated to empowering individuals recently diagnosed with type 2 diabetes. This free, accessible program offers a comprehensive 12-month journey focused on diabetes education and management, available in both English and Spanish. Participants will receive six informative e-booklets tailored to their needs, as well as a monthly e-newsletter filled with seasonal tips and local community event information. Enrollees who provide their phone number will also have the opportunity to engage in our monthly Ask the Experts Q&A series, where they can ask questions to American Diabetes Association experts, learn about the vital connection between diabetes and heart health, and enjoy giveaways designed to support their wellness journey.
