The DHHS Community Care Program, part of the Office of Population Health Equity in Houlton, ME, is dedicated to ensuring that all Mainers have access to essential health programs and social services. Through our user-friendly online referral system, we bridge the gap for individuals facing barriers in obtaining the resources they need. Our services include culturally and linguistically appropriate assistance, support for mothers and children, healthcare navigation, health and wellness education, and access to vital resources such as food, heating, and transportation. Additionally, we provide support for stress and anxiety, as well as COVID-related resources. By submitting a referral form—one per household—community members will be connected to the right organizations within 1-3 days based on their unique needs. Each resource has specific eligibility requirements and varies by region, reflecting our commitment to tailored support for every individual.
