The DHHS Community Care Program, part of the Office of Population Health Equity in Kennebunkport, ME, is dedicated to helping individuals access essential health programs and social services throughout Maine. Our online referral system is designed to assist those facing challenges in obtaining the resources for which they are eligible. By filling out our referral form, community members can connect with a wide range of support options, including culturally and linguistically appropriate services, resources for mothers and children, healthcare navigation assistance, health and wellness education, and vital resources for food, heating, and transportation. We are also equipped to provide support for stress, anxiety, and COVID-related concerns. Each referral is handled with care, and typically, one of our partner organizations will reach out within 1-3 days to discuss specific needs and available options. We believe in empowering every Mainer to access the assistance they require, recognizing that eligibility may vary by region. One referral per household is requested to ensure personalized support.
