The DHHS Community Care Program, part of the Office of Population Health Equity in Kennebunkport, ME, is dedicated to breaking down barriers to health and social services for all Mainers. Through our user-friendly online referral system, we connect individuals and families with essential resources tailored to their unique needs. Our services include culturally and linguistically appropriate support, resources for mothers and children, healthcare navigation assistance, health and wellness education, and access to food, heating, and transportation options. Additionally, we provide support for stress and anxiety, as well as COVID-related resources. By filling out our referral form, community members can receive timely connections to our partner organizations, typically within 1-3 days. We understand that each household has different needs, and we work diligently to ensure everyone receives the appropriate support, taking into account regional variations and eligibility requirements.
