The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC, is dedicated to helping residents in Kittery and throughout Maine connect with vital health programs and social services. Designed to serve everyone, especially those facing barriers to accessing available resources, our program utilizes a user-friendly online referral system. By filling out our referral form, individuals can gain access to a range of essential services, including culturally appropriate support, resources for mothers and children, assistance with navigating healthcare, health education, and options for food, heating, and transportation. We also provide resources for managing stress and anxiety, as well as COVID-related support. After submitting a referral, our compassionate staff will reach out within 1 to 3 days to discuss individual needs and connect community members with the appropriate organizations. Please be aware that eligibility requirements vary by program and region, ensuring that support is customized to meet the unique needs of each individual.
