The DHHS Community Care Program, part of the Office of Population Health Equity in Lincoln, ME, is dedicated to helping individuals overcome barriers to essential health and social services. Using an accessible online referral system, we connect Mainers to an array of resources tailored to their needs, whether it's culturally appropriate services, support for mothers and children, healthcare navigation, or health education. Our services also encompass assistance with food, heating, and transportation, as well as emotional support for stress and anxiety. By filling out a single referral form per household, community members can access a network of partners who will reach out within 1-3 days to discuss available options based on their unique circumstances. We strive to ensure that everyone can access the support they need to live healthy, fulfilling lives.
