The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC in Lisbon Falls, is committed to ensuring that all Mainers have access to essential health programs, social services, and vital resources. This inclusive program addresses the barriers many face in finding and utilizing the services they deserve. Through an easy online referral system, individuals can connect to a broad array of support, including culturally and linguistically appropriate services, healthcare navigation assistance, health and wellness education, and options for food, heating, and transportation. We also provide resources tailored for mothers, babies, and children, as well as support for mental health challenges such as stress and anxiety. By submitting a single referral form per household, community members will be matched with organizations ready to discuss their needs and available options, typically within 1-3 days.
