The DHHS Community Care Program, part of the Office of Population Health Equity in Lisbon, ME, is dedicated to connecting individuals and families with essential health programs and social services throughout the state. Additionally, we provide support for stress and anxiety, along with COVID-related resources. By completing our referral form, community members can receive tailored guidance from our staff, who will follow up with partner organizations to discuss needs and available options within 1-3 days. Each program may have specific eligibility requirements, and we encourage everyone in Maine to reach out for help in accessing the resources they need.
