The DHHS Community Care Program, part of the Office of Population Health Equity in Naples, ME, is dedicated to ensuring that every Mainer has access to vital health programs and social services. Our user-friendly online referral system is designed to help individuals and families overcome barriers in accessing the resources they need. By filling out our referral form, participants can connect with a range of essential services, including culturally tailored support, maternal and child resources, healthcare navigation assistance, health education, and options for food, heating, and transportation. We also provide resources for managing stress and anxiety as well as COVID-related support. Each household can submit one referral, and our team will work diligently to connect you with one or more of our partner organizations within 1-3 days, based on your identified needs.
