The DHHS Community Care Program, part of the Office of Population Health Equity in New Gloucester, ME, is dedicated to helping Mainers access essential health programs and social services. Our mission is to break down barriers to resource access for all residents, particularly those facing challenges. By utilizing an easy-to-navigate online referral system, we connect individuals and families with a diverse range of support services, including culturally and linguistically appropriate resources, maternal and child health assistance, healthcare navigation, health education, and options for food, heating, and transportation. Additionally, we offer support for stress and anxiety and provide resources related to COVID-19. Once you fill out our referral form, our compassionate team will work swiftly to match you with an appropriate partner organization, ensuring you receive the help you need within just a few days. To maintain effective support, we ask for one referral per household, and we note that program eligibility may vary by region.
