The DHHS Community Care Program, part of the Office of Population Health Equity in Norridgewock, Maine, is dedicated to bridging the gap between community members and essential health programs and social services throughout the state. Our online referral system is designed to assist all Mainers, particularly those facing barriers in accessing vital resources. By completing our simple referral form, individuals can connect to a diverse range of services, including culturally and linguistically appropriate support, resources for mothers and children, assistance with navigating the healthcare system, health and wellness education, and options for food, heating, and transportation. We also provide valuable support for managing stress and anxiety, alongside COVID-related resources. After submitting a referral, one of our partner organizations will typically reach out within 1-3 days to initiate a conversation about individual needs and available options, recognizing that each program has its own eligibility criteria and may vary by region.
