The DHHS Community Care Program, part of the Office of Population Health Equity in North Berwick, ME, is dedicated to bridging the gap for Mainers in need of access to vital health programs and social services. Our online referral system simplifies the process, ensuring that everyone—especially those facing barriers—can connect with the resources they are eligible for. Through our program, individuals and families can access culturally sensitive services, assistance tailored for mothers, babies, and children, and support navigating the healthcare landscape. We also provide education on health and wellness, as well as options for food assistance, heating, and transportation resources. Mental health support for stress and anxiety, alongside COVID-related resources, is available too. By submitting a referral form, households can expect a partner organization to reach out within 1-3 days for an initial conversation about their needs.
