The DHHS Community Care Program, part of the Office of Population Health Equity in Oakland, ME, is dedicated to bridging the gap between Mainers and essential health programs and social services. Our online referral system makes it easy for individuals facing barriers to access vital resources, ensuring everyone can get the assistance they need. Through our program, community members can connect with culturally and linguistically appropriate services, support for mothers and young children, guidance in navigating the healthcare system, and access to health and wellness education. We also provide resources for food assistance, heating, and transportation, as well as support for stress and anxiety and COVID-related needs. By filling out our referral form, each household can initiate a connection with our dedicated staff, who will reach out within 1-3 days to discuss options tailored to individual needs. While resources may vary by region and eligibility, our commitment is to empower every Mainer with the support they deserve.
