The DHHS Community Care Program, part of the Office of Population Health Equity in Old Town, ME, is dedicated to bridging the gap for Mainers seeking essential health programs and social services. Our mission is to empower everyone, especially those facing challenges in accessing the resources they need. Through our user-friendly online referral system, individuals can easily connect with a variety of support services tailored to their unique circumstances, including culturally and linguistically appropriate resources, healthcare navigation assistance, health and wellness education, and access to food, heating, and transportation assistance. We also provide support for stress and anxiety and offer COVID-related resources. By simply filling out our referral form—one per household—participants will receive a prompt response, typically within 1-3 days, from partnering organizations ready to discuss their needs and options, ensuring that each individual gets the help they deserve based on specific eligibility requirements.
