The DHHS Community Care Program, part of the Office of Population Health Equity in Orrington, ME, is dedicated to helping Mainers access vital health programs, social services, and resources across the state through an easy-to-use online referral system. Our initiative is inclusive, catering to individuals who face barriers in obtaining the resources they need. By filling out our referral form, you can connect with a wide range of support, including culturally and linguistically appropriate services, resources for mothers, babies, and children, and assistance in navigating the healthcare system. We offer health and wellness education, along with options for food assistance, heating, transportation resources, and support for stress and anxiety, as well as COVID-related resources. Each household is encouraged to submit one referral, after which our staff will coordinate with appropriate partners to discuss needs and available options within 1-3 days.
