The DHHS Community Care Program, part of the Office of Population Health Equity, works diligently to ensure that all residents of Maine, particularly those facing barriers, have access to essential health programs, social services, and community resources. With a focus on inclusivity, our online referral system makes it easier for individuals and families to connect with services tailored to their needs, including culturally appropriate options, maternal and child health resources, healthcare navigation assistance, health and wellness education, as well as support for food, heating, transportation, and mental health challenges. Once a referral form is completed, our dedicated staff typically connects individuals with partner organizations within just a few days, facilitating meaningful conversations about available options and support. We encourage all Mainers to reach out and take advantage of the resources available in Poland and beyond, aiming to build a healthier, more equitable community for everyone.
