The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC, is dedicated to helping individuals and families in Presque Isle and across Maine access essential health and social services. Our innovative online referral system ensures that everyone, especially those facing barriers, can connect with a variety of resources designed to meet their unique needs. By completing a simple referral form, individuals can gain access to culturally and linguistically appropriate services, health and wellness education, and assistance with navigating the healthcare system. We also provide support for mothers, babies, and children, options for food assistance, and resources related to stress and anxiety, as well as COVID-related services. Our compassionate staff works quickly to match referrals with appropriate partners, aiming to initiate contact within 1-3 days. We are here to ensure that every Mainer can find the support they need.
