The DHHS Community Care Program, part of the Office of Population Health Equity in Sabattus, ME, is dedicated to helping Mainers connect with essential health programs and social services, breaking down barriers for those in need. Our user-friendly online referral system invites individuals and families to fill out a referral form, which links them to a wide array of resources, including culturally and linguistically appropriate services, support for mothers and children, healthcare navigation assistance, health and wellness education, as well as options for food, heating, and transportation. Additionally, we provide resources for managing stress and anxiety, along with COVID-related support. Upon submitting a referral, our staff will reach out within 1-3 days to guide individuals through their options, ensuring they receive the assistance they deserve. Please note that eligibility requirements and available resources may vary by region, but every Mainer can rely on us for compassionate support and guidance.
