The DHHS Community Care Program, part of the Office of Population Health Equity in Saco, Maine, is dedicated to ensuring that all Mainers can access vital health programs and social services. Through an easy-to-use online referral system, we work to eliminate barriers that may prevent individuals from obtaining the resources they need. Our program offers culturally and linguistically appropriate services tailored for everyone, focusing on essential support for mothers, babies, and children. We provide resources to help navigate the healthcare system, along with health and wellness education, options for food assistance, heating, transportation, and mental health support. Each household can submit a referral to connect with our dedicated partners, who will reach out within 1-3 days to discuss specific needs and options. We are committed to promoting equity in health access while addressing the unique challenges faced by our community members throughout Maine.
