At the DHHS Community Care Program, part of the Office of Population Health Equity in Sanford, ME, we are dedicated to breaking down barriers to access essential health programs and social services for all Mainers. Our user-friendly online referral system enables individuals and families to connect with a wealth of resources tailored to their unique needs. From culturally and linguistically appropriate services to health and wellness education, we offer assistance navigating the healthcare system, as well as critical support for mothers, babies, and children. We also provide access to resources for food, heating, transportation, and mental health support, including stress and anxiety relief, alongside COVID-related assistance. By completing our simple referral form, community members can expect to hear back from one of our partnering organizations within 1-3 days to discuss their needs and explore options. Our mission is to ensure equitable access to vital resources, helping to foster healthier communities across Maine.
