The DHHS Community Care Program, part of the Office of Population Health Equity at ME CDC, is dedicated to ensuring that all Mainers in Scarborough and beyond can easily access vital health programs and social services. By utilizing an innovative online referral system, we aim to bridge the gap for individuals facing barriers to the resources they need. Our services are inclusive and cater to diverse needs, offering culturally and linguistically appropriate assistance, support for mothers and children, guidance through the healthcare system, health and wellness education, as well as help with food, heating, and transportation. Additionally, we provide resources for managing stress and anxiety, and specific support during COVID-19.
