The DHHS Community Care Program, part of the Office of Population Health Equity in Skowhegan, ME, is dedicated to empowering residents by helping them access essential health programs and social services. Utilizing a streamlined online referral system, our program is inclusive and designed to assist individuals facing barriers in connecting with resources for which they are eligible. By filling out our referral form, Mainers can gain access to culturally and linguistically appropriate services, support for mothers, infants, and children, healthcare navigation assistance, health and wellness education, options for food assistance, heating support, and transportation resources, as well as help with stress, anxiety, and COVID-related challenges. Once a referral is submitted—limited to one per household—our team will contact the individual within 1-3 days to discuss their needs and connect them with suitable organizations, bearing in mind that eligibility may vary by program and region.
