The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC, is dedicated to helping residents in South China, ME, connect with essential health programs, social services, and resources throughout the state. Our inclusive program is specifically designed for individuals facing barriers in accessing the care and support they need. By simply filling out our online referral form, community members can gain access to a wide range of services, including culturally and linguistically appropriate resources, assistance for mothers and children, healthcare navigation support, health and wellness education, and options for food, heating, and transportation assistance. We also offer resources to help manage stress and anxiety, along with COVID-related support. Once a referral is submitted, our knowledgeable staff will reach out to connect individuals with appropriate local organizations within 1-3 days.
