The DHHS Community Care Program, part of the Office of Population Health Equity in Standish, Maine, is committed to helping residents overcome barriers to accessing vital health programs and social services. Our user-friendly online referral system connects every Mainer with resources tailored to their unique needs, including culturally and linguistically appropriate services, support for mothers and children, guidance navigating the healthcare system, health education, and assistance with food, heating, and transportation. We also provide help for stress and anxiety, as well as resources related to COVID-19. By filling out our referral form, individuals can expect to be contacted by one of our partner organizations within 1-3 days to discuss their specific needs and options, ensuring that eligible resources are accessible to all, regardless of regional differences.
