The DHHS Community Care Program, part of the Office of Population Health Equity in Standish, ME, is dedicated to helping Maine residents access vital health programs and social services that can help them overcome barriers to necessary resources. Our welcoming online referral system allows individuals and families to connect quickly with a wide array of services tailored to their needs, including culturally and linguistically appropriate support, resources for mothers, babies, and children, guidance navigating the healthcare system, health and wellness education, and options for food, heating, and transportation assistance. We also provide support for stress and anxiety, as well as COVID-related resources. By completing a referral form, every household can initiate contact with our compassionate staff, who will connect them to partner organizations ready to assist within 1-3 days.
