The DHHS Community Care Program, part of the Office of Population Health Equity in Standish, Maine, is dedicated to connecting residents with essential health programs and social services across the state. Our user-friendly online referral system is designed to assist individuals facing barriers to accessing the resources they need. We cater to everyone, ensuring access to culturally and linguistically appropriate services, maternal and child resources, healthcare navigation support, health and wellness education, and vital options for food, heating, and transportation. Our compassionate team also addresses stress and anxiety needs, along with COVID-related resources. After submitting a referral form, community members can expect to be contacted by one of our partner organizations within 1-3 days to discuss their unique needs and options.
