The DHHS Community Care Program, part of the Office of Population Health Equity in Topsham, ME, is dedicated to assisting residents in navigating critical health programs and social services throughout the state. Our online referral system is crafted for individuals facing challenges in accessing the resources they're entitled to, ensuring equitable support for all Mainers. By completing our referral form, individuals gain access to a range of services, including culturally and linguistically appropriate assistance, resources for mothers and children, healthcare navigation support, health and wellness education, food assistance, heating and transportation resources, mental health support for stress and anxiety, and COVID-related services. After submitting a referral, our staff will connect community members with partner organizations, typically within 1-3 days, to discuss their needs and explore available options. We are here to empower every individual and family with the resources they require, tailored to their specific circumstances and regional needs.
