The DHHS Community Care Program, part of the Office of Population Health Equity in Turner, ME, is dedicated to bridging the gap between Mainers and vital health programs, social services, and essential resources throughout the state. Designed to assist individuals facing barriers in accessing necessary services, this inclusive program invites everyone to submit a referral form. Through this streamlined process, participants can connect with a range of supports, including culturally appropriate services, resources for mothers and children, healthcare navigation assistance, health and wellness education, as well as food, heating, and transportation options. Additionally, we provide support for stress and anxiety and COVID-related resources. Typically, one of our partnering organizations will reach out within 1-3 days to discuss specific needs and available options. It's important to note that eligibility requirements vary by program and region, ensuring that services are tailored to the unique circumstances of each community member.
