The DHHS Community Care Program, part of the Office of Population Health Equity in Union, ME, is dedicated to empowering Mainers by connecting them with essential health programs and social services throughout the state. Our mission is to ensure everyone has access to the resources they need, especially those facing barriers. Through our online referral system, individuals can easily fill out a form to receive assistance tailored to their unique situations. We offer a range of services, including culturally and linguistically appropriate support, resources for mothers and children, healthcare navigation, health and wellness education, as well as options for food, heating, and transportation assistance. Additionally, we provide crucial support for stress, anxiety, and COVID-related challenges. After a referral is submitted, our caring staff will begin the process of linking community members with appropriate organizations, typically within 1-3 days. We strive to meet the diverse needs of our community while recognizing that eligibility for resources may vary by region.
