The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC in Winterport, ME, is dedicated to connecting Mainers with essential health programs and social services to overcome barriers to resources. Utilizing a convenient online referral system, we make it easy for individuals to access a wide range of support, including culturally and linguistically appropriate services, maternal and child health resources, healthcare navigation assistance, health and wellness education, and options for food, heating, and transportation. Our program also offers support for stress, anxiety, and COVID-related challenges. By submitting a referral form, households can initiate connections with our partner organizations, who will generally reach out within 1-3 days to discuss available options tailored to their needs. We strive to ensure that every Mainer receives the assistance they deserve through our comprehensive network of resources.
