The DHHS Community Care Program, part of the Office of Population Health Equity in Winthrop, ME, is dedicated to bridging gaps in access to essential health and social services for all Mainers. Utilizing an online referral system, our program is designed for individuals who face barriers in connecting with the resources for which they are eligible. We invite you to fill out our referral form to access a wide array of supports, including culturally and linguistically appropriate services, health and wellness education, and assistance with food options and transportation. Whether you need help navigating the healthcare system, resources for mothers and children, or support for stress and anxiety, we are here to help. Following your referral, our compassionate staff will connect you within 1-3 days to a partner organization that can discuss your needs and explore solutions. Each program has specific eligibility requirements, so we encourage you to reach out and discover the resources available to you in your region.
