The DHHS Community Care Program, part of the Office of Population Health Equity at the Maine CDC in Wiscasset, is dedicated to empowering Mainers by facilitating access to essential health programs and social services. Our user-friendly online referral system is designed for individuals who may face challenges in obtaining the resources they need. By completing our referral form, participants can connect to an array of services tailored to various needs, including culturally and linguistically appropriate resources, healthcare navigation support, health education, and assistance with food, heating, and transportation. We also provide resources for mothers, children, and those coping with stress, anxiety, or COVID-related concerns. Our committed staff will respond within 1-3 days to discuss options, ensuring that every individual receives the attention and support they deserve.
