The DHHS Community Care Program, part of the Office of Population Health Equity at ME CDC, strives to empower individuals in York, ME by providing easy access to vital health programs and social services. Our online referral system is designed for everyone, particularly those facing barriers to obtaining the support they deserve. By filling out our referral form, you can connect with a diverse range of resources including culturally and linguistically appropriate services, support for mothers and children, healthcare navigation assistance, health and wellness education, and essential resources for food, heating, and transportation. We also offer help with stress and anxiety as well as COVID-related resources. Each referral is handled with care, and one of our partners will reach out within 1-3 days to discuss your unique needs and options.
