Located in Trenton, MI, Cancer Transitions is a dedicated program designed to support survivors of any cancer diagnosis who have completed their treatment. While our primary focus is on individuals who have finished treatment within the last 24 months, our resources benefit survivors at any stage of their healing journey. We offer comprehensive services, including nutritional support to aid in recovery, customized exercise plans tailored to individual needs, as well as emotional health education and medical management support. Our mission is to empower cancer survivors with the tools and knowledge they need to thrive post-treatment, fostering a community of resilience and well-being.
