Located in Chanhassen, MN, the Living With Type 2 Diabetes Program is dedicated to supporting individuals recently diagnosed with type 2 diabetes. This free, comprehensive program bridges knowledge gaps over a 12-month journey, providing participants with six essential e-booklets tailored to their needs, alongside a monthly e-newsletter filled with seasonal tips and resources available in both English and Spanish. Participants also gain access to local community events and, by providing their phone numbers, are invited to the monthly 'Ask the Experts' Q&A series, where they can connect directly with American Diabetes Association experts to address their questions, discover the connection between diabetes and heart health, and participate in engaging giveaways aimed at enhancing their health and well-being.
