The Living With Type 2 Diabetes Program in Pine City, MN is dedicated to supporting individuals recently diagnosed with type 2 diabetes through a comprehensive and free 12-month educational initiative. We provide essential resources, including six informative e-booklets tailored to meet participants' needs, a monthly e-newsletter filled with seasonal tips and community event information, all available in both English and Spanish. Enrollees have the opportunity to engage directly with diabetes experts during our monthly Q&A sessions, where they can ask questions, learn about the connection between diabetes and heart health, and receive helpful giveaways. Our mission is to empower participants with the knowledge and tools they need to effectively manage their diabetes and thrive in their journey towards better health.
