Located in Thief River Falls, MN, Cancer Transitions is a dedicated program crafted for cancer survivors who have completed their treatment. While our services are primarily tailored for those who have finished treatment within the last 24 months, we welcome and support survivors at any stage of their journey. We focus on providing essential resources, including nutritional support, customized exercise plans, education on emotional health, and guidance on medical management.
