Over a supportive 12-month period, participants engage with tailored content, including six informative e-booklets designed to meet them at their unique starting points in diabetes management. The program enriches their journey with a monthly e-newsletter featuring seasonal tips and local community event information available in both English and Spanish. Enrollees can also enhance their experience by submitting their phone numbers to join our monthly 'Ask the Experts' Q&A series, allowing them to pose diabetes-related questions to professionals from the American Diabetes Association, learn about the critical relationship between diabetes and heart health, and benefit from helpful giveaways.
