The Living with Type 2 Diabetes Program, located in Hickman, NE, is dedicated to supporting individuals recently diagnosed with type 2 diabetes through a comprehensive and free 12-month journey towards effective diabetes management. Participants, available in both English and Spanish, benefit from six informative e-booklets, each tailored to their needs, as well as a monthly e-newsletter filled with seasonal tips and community resources. Our program fosters a sense of connection and learning by inviting enrollees to submit their phone numbers for exclusive access to our monthly 'Ask the Experts' Q&A series, where they can engage with ADA experts, explore the critical link between diabetes and heart health, and enjoy helpful giveaways designed to enhance their diabetes management. Together, we strive to empower individuals to live healthier lives with type 2 diabetes.
