Located in Minden, NE, the Living with Type 2 Diabetes Program is dedicated to empowering individuals recently diagnosed with type 2 diabetes through a comprehensive, free 12-month journey of education and support. Participants receive six informative e-booklets tailored to their needs, along with a monthly e-newsletter filled with seasonal tips and community resources available in both English and Spanish. The program also offers valuable opportunities to engage with diabetes experts during a monthly Q&A series, where enrollees can ask questions and gain insights about managing diabetes and its connection to heart health. Our goal is to equip participants with the knowledge and tools they need to thrive and navigate their diabetes with confidence.
