The Living with Type 2 Diabetes Program, based in Contoocook, NH, is dedicated to supporting individuals recently diagnosed with type 2 diabetes by providing free, comprehensive education and resources over a structured 12-month period. This program features six informative e-booklets tailored to guide participants through essential topics in diabetes management, alongside a monthly e-newsletter filled with seasonal tips and local community event information, available in both English and Spanish. Enrolled members can also take advantage of our monthly Ask the Experts Q&A series, where they can interact with diabetes specialists from the American Diabetes Association, ask pertinent questions, and gain valuable insights on managing their condition, including the link between diabetes and heart health. We believe in empowering those affected by diabetes with the knowledge and tools they need to thrive.
