The Living with Type 2 Diabetes Program in Enfield, NH, is dedicated to supporting individuals newly diagnosed with type 2 diabetes through a comprehensive and free educational initiative. Over the course of 12 months, participants will engage with six practical e-booklets tailored to their learning needs, alongside a monthly e-newsletter filled with seasonal tips, resources, and information about local community events, all available in both English and Spanish. Enrollees who provide their phone number will gain access to the monthly 'Ask the Experts' Q&A series, where they can pose diabetes-related questions directly to ADA experts, explore the connection between diabetes and heart health, and enjoy giveaways designed to promote their well-being. Our mission is to empower our participants with the knowledge and resources needed to manage diabetes effectively and confidently.
