Cancer Transitions is a compassionate program based in Hillsborough, NH, dedicated to supporting survivors of all cancer diagnoses who have recently completed their treatment. Specifically designed for those in the first 24 months post-treatment, our program offers invaluable resources that extend beyond this timeframe. We focus on four key areas of support: nutritional guidance, individualized exercise plans, emotional health education, and medical management assistance.
